Prelim 2 Alternate Time Request

Prelim 2 is scheduled for Tuesday, April 19 @ 7:30 PM.

Please complete this form if you:

• would like to request a makeup exam time because you cannot take the 7:30pm exam due to a conflict or other reason. There are 28 courses with prelims on April 19! Not sure if you have a conflict? Please look up all your Spring 2022 classes on the Spring 2022 Prelim Schedule and see if any collide with the CS 1110 prelim. 
• would like to use an SDS exam accommodation that concerns more than extended time/breaks/a quiet location
• have an SDS exam accommodation that we are unaware of (i.e., not confirmed by your score on Extended Time Exam Accommodations)
• expect an SDS letter but it is not yet filed with the instructor(s)

You do not need to complete this form if:

• when you look at your score on Extended Time Exam Accommodations you see that we already plan to provide you a quiet room and/or extra time or breaks
• can take Prelim 2 as scheduled

Makeup exams may be requested by students with conflicts, although we reserve the right to deny makeups for non-legitimate conflicts or for students who fail to follow our directions. (This has, unfortunately, happened in the past.) Requests stemming from University-imposed conflicts are certainly considered legitimate.

Canvas may list this as a graded survey but that is only because we do not want your responses anonymized. Rest assured this "assignment" does not count toward your semester grade!
